HOW TO DESIGNATE YOUR CHOICE DOLLARS:
Online:
1. Visit www.thrivent.com/thriventchoice. Click on “log in” at the top right of your screen and enter your User ID and password. (If you haven’t yet registered, click “Register Now”)
2. Look for your designated Choice Dollars amount on the right side of the screen.
3. Click on “Direct Choice Dollars” button.
4. Enter “Lutheran Social Services of New York” in the box.
5. Click the “Direct Now” button, then enter your desired amount.
By Phone:
1. Call 800.847.4836 and when prompted, say “Thrivent Choice.”
2. Say “Choice Dollars,” then press 1 to direct Choice Dollars (a representative will assist you).
